We are dedicated to helping our low income community allow their senior or medically ill pets to cross the rainbow bridge with dignity & peace. This program is strictly for low income families.
Fill out the form below and provide proof of low / no income directly on the form. We will be in touch as soon as possible.
1.) The vet clinic must approve this as a medically necessary passing. We will not, under any circumstance, approve any requests dealing with behavior, training, biting, etc.
2.) HSO & Vet Clinics reserve the right to refuse this program or stop the program at any time.
3.) The program is only set up with limited veterinarians. You must select one from the list on the form.
4.) You will not be allowed to retrieve the ashes from your pet.
5.) Limited to 1 voucher per 12 months (not calendar years, but 12 months from the date of approval)
